The Vermont Lead Law Disclosure – Single-Family Home form is used when an owner-occupied single-family home built before 1978 is being sold and the property is subject to an Assurance of Discontinuance, Administrative Order, or Court Order related to lead hazards that has not yet been completed. The seller must disclose the existence of the order and provide a copy to the buyer before the purchase and sale agreement is executed or at the time of sale. This requirement is part of Vermont’s Lead Poisoning Prevention Law (18 V.S.A. Chapter 38), which regulates lead hazards in older housing. The completed form must also be submitted to the Vermont Department of Health.
